1. Order /and/ for the TAD released /original + copy/.
2. Form, filled in with order and model chosen.
3. Personal data: ID.№, date, MIA and PIN /or copy of the identity card/

The documents, quoted above, could be sent by a delivery company on the “Health for everyone” association expenses. Your order will be processes within two-days period and the respective TAD, delivered to the address quoted on the expenses of our organization.
